Scheduling a New Patient Appointment

To schedule your pet’s new patient appointment, and to provide your pet the best care possible, please complete the following steps:

  1. Please call your primary care veterinary clinic and ask them to share your pet's full medical records with ADC-Austin. Your veterinarian can provide the records to us in several ways:
    • Via the Refer Patient found on our website
    • By email to infoaustin@adcmg.com
    • By fax to 512-477-4826
  1. Please complete and submit the following information:
    • Our New Patient Form
    • Our New Client Form

We will contact you to schedule your pet’s appointment once all your pet’s medical information is received.

A credit card is required at the time your pet’s appointment is scheduled. This information will be stored in our safeguarded system. A charge for a missed exam fee will only take place in the event of a “No call, No show” or a cancellation with less than 24 hour notice (or the Friday before your appointment if your appointment is on a Monday.) Your stored credit card may also be used to check out at future appointments or for medication refills.

We are making every effort to shorten the wait times for appointments; however, we are still booking weeks in advance and appreciate your patience. We advise you to continue the treatment plan from your regular veterinarian in the meantime.
